Scheduling a Meet and Greet is an excellent opportunity for prospective parents and new patients to ask questions, get to know our approach, and meet one of our caring providers. We’re here to support you in making the best healthcare decisions for your family.

When Transferring to Our Office...
- Register for our Patient Portal. This will create a medical chart for your child in our system and make scheduling appointments quick and easy.
- Complete a records release form. You can do this either here at our office or through your previous healthcare provider, ensuring we have all the important information to provide the best care for your child.
Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your children's medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.
Our Goal Is To Provide The Best Medical Care
To achieve this, it’s important for us to review your child’s complete medical history from all previous healthcare providers.
Once we have received and reviewed these records, we will reach out to you to schedule your child’s first appointment.
